Welcome to the Flag Request Form

An unofficial tool to quickly generate the AOC's flag form.

How to use this form:

1. First-time setup: Fill out all fields that will remain constant (office name, staffer, room, phone, mailing info). Leave the constituent name blank.
2. Save your defaults: Click "Save Defaults" to store these defaults in your browser.
3. Daily usage: Fill in the constituent name and any other variable information, then click "PRINT".
4. Sharing with coworkers: Use "Export" to copy defaults, then "Import" on another computer to transfer your setup. This will transfer all defaults, including the signature.
